From May 1 to June 4, Costco employees will be asking members to donate at checkout and 100% of every donation will support your local children’s hospital foundation. Costco Members who donate $20 or more are eligible for a charitable tax receipt issued by your local Children’s Miracle Network hospital foundation. Click on the button above to request a tax receipt. Do not fill out an online form if you completed a paper request in the warehouse. Please submit tax receipt request by June 30, 2023.
