From May 1 – June 1, 2025, Costco employees will be asking members to donate at checkout. 100% of every donation will support your local children’s hospital foundation. Donations made at your Costco Warehouse or online help kids like Kaitlynn get the best care and treatment possible. Watch Kaitlynn’s story.

Costco Members who donate $20 or more are eligible for a charitable tax receipt. Tax receipts are issued by your local children’s hospital foundation once all campaign funds have been reconciled, by March 2026. Please do not fill out the online form below if you completed a paper request in the warehouse.
Please submit all tax receipt requests by June 30, 2025.
